Summary

Martin Avenue Pharmacy, Inc. is voluntarily recalling approximately 1,360 units of all in-date compounded sterile veterinary preparations, including Peng/proc/strep/chlor/hyos, Phenylephrine 1%, Prednisolone acetate, "Tri-mix", and Tacrolimus oil. These medications are being recalled following an FDA inspection that identified quality control issues that could compromise the sterility of the products. These sterile medications were distributed nationwide to several states in amber or clear glass vials and squeeze bottles.

  2. Immediately stop using any of the affected medications for your animals.
  3. Contact your veterinarian or pharmacist to discuss alternative treatment options and to report any adverse health events observed in your pets.
  4. Return any unused portions of these medications to the pharmacy where they were purchased for a refund and further instructions.
