Summary

Approximately 2,590 high pressure scuba diving air hoses sold under the Phantom and Mesh Flex brand names have been recalled. The black braided hoses, which range in length from 6 to 40 inches, can rupture while in use. The importers have received nine reports of these hoses bursting during operation, although no injuries have been reported. Consumers should stop using the affected hoses and contact the respective manufacturer for a free replacement.

Risk

The diving hose can rupture or burst, causing a sudden loss of air and potentially leading to a drowning hazard for the diver. While no injuries have been reported, nine separate incidents of hose failure have been documented.

What You Should Do

  1. Check the metal fitting of your scuba hose for the following production date codes stamped into the metal: Innovative Scuba Concepts codes 11Q3, 11Q4, and 12Q1; Trident Diving Equipment codes T0811, T0911, T1011, T1111, T1211, T0112, and T0212; A-Plus Marine codes 11Q3, 11Q4, and 12Q1.
  2. Verify the brand name on the original product packaging, as the hoses were sold under the names 'Phantom' and 'Mesh Flex'.
  3. Immediately stop using any hose identified with these production codes.
  4. Contact the company you purchased the hose from to receive instructions for a free replacement: Innovative Scuba Concepts at (800) 472-2740 or www.innovativescuba.com; Trident Diving Equipment at (800) 234-3483 or www.tridentdive.com; or A-Plus Marine at (800) 352-2360 or www.aplusmarine.com.
