Flushmate Recalls Flushmate® III Pressure-Assisted Flushing System Due to Impact and Laceration Hazards

Summary

Approximately 2.3 million Flushmate® III Pressure-Assist flushing systems are being recalled after more than 300 reports of the units bursting inside toilet tanks. These systems, found in brands like American Standard, Kohler, and Gerber, can suddenly release stored pressure at the weld seam, causing the porcelain toilet tank to shatter or the lid to be forcefully ejected. Flushmate has received reports of 14 injuries involving impact or cuts from broken porcelain. Consumers should immediately stop using these systems, turn off the water supply, and contact Flushmate for a free repair kit.

  2. Locate the 16-character serial number on the label on top of the Flushmate unit; this recall affects units with date codes (the first six digits) ranging from 101497 (October 14, 1997) through 022908 (February 29, 2008).
  3. If your unit is affected, immediately turn off the water supply to the toilet and stop using the system until it is repaired.
