Summary

Approximately 453,000 Flushmate® III Series 503 Pressure-Assist Flushing Systems are being recalled because the internal vessel can burst at the weld seam. These systems are rectangular black plastic units installed inside toilet tanks. The company has received two reports of units bursting, resulting in property damage and one minor injury. Consumers should immediately stop using these systems, turn off the water supply, and contact Flushmate for a free repair kit.

Risk

The pressure-assist vessel can burst under pressure, causing the toilet tank lid to lift or the porcelain tank itself to shatter. This creates a serious risk of impact and laceration injuries to consumers as well as potential property damage.

What You Should Do

  1. Check the label on the top of the black Flushmate III vessel inside your toilet tank to identify the 16-character serial number. Affected units have a serial number starting with a six-digit date code between 070109 (July 1, 2009) and 043011 (April 30, 2011).
  2. Confirm if your unit was manufactured by American Standard, Copperfit, Crane, Ecotech, Eljer, Gerber, Kohler, Lamosa, Mancesa, Mansfield, Orion, St. Thomas, Universal Rundle, Vitra, Vitromex, Vortens, or Western Pottery.
  3. Immediately stop using the recalled Flushmate III system and turn off the water supply to the toilet.
  4. Flush the toilet once the water is off to release any remaining internal pressure.
  5. Contact Flushmate at 800-303-5123 between 8 a.m. and 10 p.m. ET Monday through Friday, or 8 a.m. and 6:30 p.m. ET Saturday, or visit www.flushmate.com and click on 'Flushmate III Recall' to request a free repair kit.
  6. Call the CPSC Hotline at 800-638-2772 for additional questions.
