Flushmate Expands Recall of Flushmate III Pressure-Assisted Flushing System Due to Impact and Laceration Hazards

Approximately 351,000 Series 503 Flushmate III Pressure-Assist flushing systems are being recalled because the internal vessel can burst at or near its weld seam. This release of stored pressure is powerful enough to lift the toilet tank lid and shatter the ceramic tank, which has resulted in three reports of property damage. These rectangular black plastic units were sold individually and installed in toilets from brands including American Standard, Kohler, and Mansfield between 2008 and 2009. Consumers should stop using the recalled systems and contact the manufacturer for a free repair kit.

Risk

What You Should Do

  1. Check the top of the black Flushmate III vessel inside your toilet tank for a label containing a 16-character serial number.
  2. Verify if your unit is affected by checking the first six digits of the serial number (the date code); recalled units have date codes ranging from 030108 (March 1, 2008) through 063009 (June 30, 2009).
  3. Check if your system was part of the previous recall which included date codes from 101497 (October 14, 1997) through 022908 (February 29, 2008).
  4. If your unit is recalled, immediately turn off the water supply to the toilet and flush it once to release the internal pressure.
  5. Stop using the toilet until the system is repaired.
